Health Services Survey Open to Chemung Co. Residents

ELMIRA, N.Y. (WENY) -- Chemung County is working to create a healthier place to live, and officials are asking for the public's help. 

The Health Department is asking for residents to complete an online survey to gather information and data about various health services and concerns throughout the county. 

It is expected to take about 15 minutes. The project allows people to share their experiences with health services in the community. To access the survey, go to MyHealthStory2018.

“Your personal health story can help improve lives. It is important for us to hear from as many residents as possible in order to plan our future public health initiatives and to direct the focus of our activities,” stated Peter Buzzetti III, Chemung County’s Public Health Director.

It can be done anonymously or, if you enter your name and email, you'll be entered to win a $500 gift card to a local supermarket or gas station.
